The Alder Associates team of specialist paediatric case managers are experts in the provision of case management packages tailored specifically to the needs of children. Thanks to our small, professional team’s 40 years of combined experience, we can support both paediatric clients and their families with a wide variety of health issues, disabilities and injuries, including those with complex needs.

Our Case Management Process

We know how important it is to our clients and their families to ensure that assessments are carried out quickly so that interventions can rapidly be put in place in order to facilitate recovery. That’s why we’ve developed a streamlined process which runs as follows:

We receive a referral or enquiry about our services.

We provide an Immediate Needs Assessment quote and pinpoint the best-suited paediatric case manager on our team to work with the family.

The Immediate Needs Assessment (INA) takes place.

The assigned case manager draws up a comprehensive report outlining proposed rehabilitation and case management package.

Case management recommendation approval will be sought and agreed

Case management, rehabilitation and support can commence

Our Services Include

  • Carrying out an Immediate Needs Assessment to compile a report that outlines costings and recommendattions.
  • Liaising with appointed deputies and solicitors that are involved in Court of Protection and litigation processes.
  • Comprehensive planning for rehabilitation care including highly-detailed risk assessments.
  • Recruitment and induction of teams of support workers as well as their support and training.
  • Planning and implementation of client-specific goals.
  • Assisting in research and then securing appropriate therapeutic services provision.
  • Supporting the implementation, transitions and review of EHCPs
    (Education and Health Care Plans).
  • Attending meetings and appointments
    with the child and their family or on their behalf.
  • Organising multi-disciplinary meetings so a seamless approach can be achieved across services.
  • Supporting families with financial issues and accessing benefits.
  • Advocating for the family and child.
  • Sourcing appropriate accommodation, vehicles and equipment to meet the needs of the child.

Frequently Asked Questions

A paediatric case manager plans, coordinates, rehabilitates, supports and cares for your child after they have suffered from a severe or complex injury.

Your case manager will visit you at home and will discuss in great detail your child’s needs as well as those of your wider family. They will take into account all your concerns and worries and will detail them in a full report which will then be used as a basis for a rehabilitation and support package.

An INA is an Immediate Needs Assessment. This is a comprehensive report which is compiled by your child’s case manager in conjunction with you and your wider family. It includes costings and recommendations for your child’s ongoing support, rehabilitation and care.

Your child’s case manager will liaise with appointed deputies and solicitors that are involved in COP (Court of Protection) and litigation processes.

Part of your case manager’s remit is to help you to research and source all the necessary equipment, vehicles and accommodation your child needs to enjoy the best quality of life by working closely with qualified suitable therapists.
